Implications for Clinical Practice
Reminding patients to continue with the plan of care is one way to keep patients healthy. The patient portal reminder system is one method for improving communication with patients. This project will help the organization adopt ways to improve the patient portal reminder system to increase adherence to the PrEP program and increase the re-testing rate for patients diagnosed with STIs. The limitation of the portal use includes not having email access, and also, some patient requires training on how to access the portal system from their device so they can access the portal quickly. The patient portal reminder is one way of communicating with patients; however, there is a limitation pt have to have email access to utilize the portal system. Though some patients know how to access the portal system, each patient visit staff and providers need to remind patients to educate patient how to access it. In order to encourage patients to be more involved in patient reminder systems, apply incentives for those who use the portal system for communication.
Implications for Health Policy
The public health policy in the United States supports the universal prevention of HIV and STIs. TGWs are underrepresented; health disparities exist in the LGQBT population, especially in the TGW; therefore, a healthcare policy is needed to address the current healthcare issue. Digital access, such as patient portal systems, requires email access, and politicians and policymakers need to advocate support and adapt access to a digital device for the vulnerable population.
Healthcare policymakers should allocate funds and work with underserved communities. Providing digital services for the community will allow them to communicate with the clinic. Not having a permanent address, not having a stable income, and owning an electronic device with email access are more challenging and add burden to this serving population. Providing electronic devices with email access would be an excellent intervention to improve communication between the patients and the clinic will improve the quality of care.
Implications for Quality & Safety
The Patient portal reminder system impacts adherence for the patient in the PrEP clinic; however, there are some safety concerns regarding PrEP use. The most significant impact of using PrEP in the high-risk population is to decrease the incidence of HIV infection. However, the retention and uptake of PrEP raise some significant challenges. Some TGWs decline to take PrEP due to the interaction of the medication with hormone therapy. However, the main reason we need to consider safety for the patient taking PrEP is due its side effects on the body, especially the Liver. There is a need for patient education to report any change in the body for possible side effects, like nausea, vomiting and headache, and possible diarrhea. It is important to ask patients if they are experiencing loss of appetite, light-colored stool, and dark-colored urine. In severe cases, it can affect, Liver and kidneys and cause loss of bone density.
Educational Implication
Patient education on how to use the patient portal reminder system is vital; some patients have access to the patient portal but cannot navigate the browser. The healthcare provider and the team must educate patients on using the portal system. In addition, it is imperative to educate patients on the effectiveness of PrEP in preventing HIV; however, there is a need to use condoms to prevent other STIs. Educating patients during each visit screening for STI is as important as adherence to PrEP to decrease the incidence of HIV and STI (GC/CT).
Economic Implications
Regarding economic impact, it has been estimated that the lifetime cost to treat new STI cases was $15.9 billion in direct medical costs, with HIV accounting for nearly $13.7 billion (Chesson et al., 2021). Studies examining the cost-effectiveness of HIV PrEP for high-risk populations have been favorable, supporting the use of PrEP as a cost-effective means of preventing HIV infection (Adams et al., 2019).
Sustainability
The data in this review supports the need for PrEP care and STI screening in the TGW population since this is the first QI project that focuses on TGW compared to the previous study that includes the general population. Though the finding is sufficient to show that the Portal system is a effective tool to improve adherence to PrEP care and increase STI screening, it was noted that the follow-up rate needed to be more consistent. There is a need to improve the patient portal reminder system to improve outcomes. The site is a research center, and more funding and resources are needed to support the existing PrEP program and focus on improving the retention rate in the PrEP program and the re-testing rate of STI in the TGW population.
Professional Reporting
The result of the project disseminated within the Rutgers School of Nursing will be presented during the poster day via a poster presentation. The finding will be shared with the institution's stakeholders as well. The co-investigator anticipates the project findings to be published in the nursing journal and presented at the nursing association conference.

Conclusion
The impact of using the patient portal system on patients who receive PrEP care is to increase adhere to care. The patient portal system used on TGW who are diagnosed with STI is to increase the rate of retesting every three-month follow up as recommended. For this QI project the focus was on STI just GC/ CT Syphilis was not included. However during the chart review it was observed that TGW who diagnosed with syphilis. Due to the finding there is a need to address other STI in the TGW population. Patient portal system was effective to communicate with patients.
