4. An otherwise healthy 15 year old female skateboarder fell while riding, which resulted in a right clavicle fracture.  The fracture was reported with internal fixation under general anesthesia, which was administered by the anesthesiologist. 

 

5. An 85 year old female was admitted to the hospital for metastatic bone cancer.  The primary site of cancer is unknown.  A malignant lesion was discovered on her right third rib.  She underwent a partial rib resection under general anesthesia, which was administer by the anesthesiologist. 

 

6. A 74 year old with symptomatic atrioventricular blockage and permanent pacemaker implantation developed Twiddler syndrome two weeks after implantation of the pacemaker.  The pacemaker was replaced under general anesthesia, which was administered by the anesthesiologist. 

 

7. A type 1 diabetic 82 year old female with a history of tobacco abuse was dx with a malignancy on her esophagus.  She underwent a total esophagectomy under general anesthesia, which was administered by the anesthesiologist. 


8. An otherwise health 43 year old male was dx with a benign thoracic spinal tumor.  He underwent a neurological resection under regional anesthesia, which was administered by a CRNA who received medical direction from the physician. 

 

9. An obese 57 year old male with type 2 diabetes was dx with recurring upper back pain due to displacement of cervical intervertebral disk.  He underwent posterior extradural laminotomy for decompression of the cervical spine, under regional anesthesia,  which was administered by the anesthesiologist. 

 

10. The patient received anesthesia for laparoscopic cholecystectomy.

 

11. Anesthesia for bilateral vasectomy.

12. Anesthesia for tracheobronchial reconstruction.

13. Daily hospital management of epidural, continuous drug administration.

14. An 81-year-old patient with mild systemic disease who receives anesthesia for revision of total hip arthroplasty.

15. A 9-month-old normal child who received anesthesia for hernia repair in the lower abdomen.
