She is on a cardiac diet with a 2-g sodium restriction. The nurse assesses the client, noting the vital signs: T, 98° F; BP, 150/80 mm Hg; HR, 80 beats/minute; RR, 24 breaths/minute. The client has an aortic systolic murmur heard after the S1, and the point of maximal impulse is at the 6th intercostal space (ICP). The lungs are clear anteriorly and posteriorly.
