After getting the appropriate consent from the patient, under general anesthesia, the patient was placed in the left lateral decubitus position with the table flexed at 30 degrees. A 10 mm zero-degree thorascope was inserted into the right plural cavity through a port site placed in the sixth intercostal space on the midaxillary line. Two additional port were placed at the 5th intercostal space on the posterior and anterior midaxillary line for consideration of a possible thoracotomy. The VATS exploration revealed a mass in the base of the right lung. The mass was wedge biopsied and sent to pathology for frozen section. Results showed small cell carcinoma. The entire mass was subsequently resected - thoracoscopic resection of the RLL and sent to pathology. Inspection of the lung revealed normal parenchyma. The port sites were closed and the patient was extubated and transferrred to surgical intensive care for postoperative observation.
