Nancy is a 20-year-old female seeking individual therapy. During her initial appointment, she explained that she is dealing with anxiety and depression, which has interfered with her ability to succeed in college and maintain close interpersonal relationships due to her anxiety and depression. Nancy explained she was raised by her grandmother who mistreated her by calling her all sorts of names, including "fat," "ugly," and "dumb." She grew up very shy and always had difficulty making friends in grade school. Nancy noted her mother was a single parent and was always busy working, therefore, she wasn't always present or very affectionate with her. She is having difficulty concentrating in school and very anxious when she is around classmates and specially during presentations, as she tried to avoid these because she thinks she is "not smart enough." She is seeking therapy because she wants to improve her mood, be less socially anxious, and establish friendships.
