TJ, a 32-year-old pregnant lesbian, is being seen for an annual physical exam and has been having vaginal discharge. Her pregnancy has been without complication thus far. She has been receiving prenatal care from an obstetrician. She received sperm from a local sperm bank. She is currently taking prenatal vitamins and takes over the counter Tylenol for aches and pains on occasion. She has a strong family history of diabetes. Gravida 1; Para 0; Abortions 0
Cultural competence refers to the ability to overcome cultural barriers between patients and health care providers (HCP), allowing the HCP to meet the needs of patients with different cultural backgrounds (Ball, Dains, Flynn, Solomon, & Stewart, 2019). Awareness of the issues faced by Lesbian, gay, bisexual, transgender, and questioning/queer (LGBTQ) enables us to reach out to these patients to provide excellent care.
Socioeconomic, Spiritual, Lifestyle & Cultural Factors
Women who self identify as lesbian encounter barriers to health care that include concerns related to maltreatment, confidentiality, discrimination, stigma, limited access to health insurance and, lack of understanding of health risks due to their sexual orientation, gender or expression (Kates, Ranji, Beamesderfer, Salganicoff & Dawson, 2018). Lesbians face the same health issues as heterosexual women. However, they also face some significant additional health risks. (Kates et al., 2018). TJ seems to be on the right path and well educated about seeking care for her and her unborn child, but may need additional wellness exams and education on preventing diseases. By obtaining a full history, the HCP can identify the patient’s health-related practices and appropriately target screening, prevention, counseling, and referrals based upon her needs (Dains, Baumann, & Scheibel, 2019).
Sensitive Issues
To ensure quality care, all patients, regardless of personal or cultural backgrounds, should be treated with dignity and respect and should feel comfortable providing sensitive information relevant to their treatment plans, including information about sexual orientation. Assess the patient’s sexual history and practices making sure to use a nonjudgmental approach to ensure the patient is comfortable and honest (Ball et al., 2019).
Reassure the patient that some topics discussed during the interview may be sensitive and too personal but are necessary to obtain information on what may be the cause of symptoms to provide optimal care.

Who is your support system during pregnancy and after childbirth?
