Case Scenario: Mary history: Mary has a risk of being hospitalized due to mental health. Mary has a history of isolation and depression. Mary symptoms includes mood swings, anger outburst, and isolation and feeling hopelessness. Mary needs reminders to take her medications. Mary recently missed two scheduled appointments with her psychiatrist and eye doctor. Mary currently lives with her daughter and 3-grnadchildren

Strategies:

1. Social worker will educate Mary about her diagnosis. Social worker will engage Mary on exploring and identifying daily symptoms. Social worker will explore effective coping skills through role-play. Social worker will coach and model and demonstrate how to use coping skills. Social worker will engage Mary in positive thinking activities.

2. Social worker will review the benefits of creating a schedule as it relates to independence living skills. Social worker will support Mary in determining activities to include in her schedule. QMHP will modification of schedule as needed.

3. Social worker will utilize Mary’s genogram to explore her family pattern. Social worker will encourage Mary to explore the relationship between her family patterns and mental health.
