I have provided the process recording templet and the intro to Mrs. O this is our second visit and she has been to see her doctor for the assessment needed to apply for DSHS but he was supposed to fax it to DSHS and did not so I sat with her and called his office. We were told he was an vacation but I said his nurse can fax it . They said we needed to talk to medical records but then medical records sent us to the doctor covering for her primary doctor.
