PATIENT-CENTERED EVIDENCE-BASED PRACTICE POST
The healthcare industry has become more complex; therefore, healthcare workers must provide more efficient and effective healthcare to address their needs fully. The patient-centered approach was proposed by the IOM "institution of medicine" involving a more responsive and open-minded approach where the values and needs of each patient are addressed through an evidence-based decision-making process. The patient-centered concept represents several factors that positively improve the interactions between the healthcare providers and the patients, ensuring that the patient's unique needs are met and addressed. However, for patient-centered care to be achieved, there should be constant communication, sensitization, and education at all treatment phases. Additionally, through a patient-centered approach, the healthcare workers become more committed to their work where each patient's needs are addressed with dignity, respect, and humane without discriminating against the patients. Previously patients were highly discriminated especially based on their abilities to pay for healthcare insurance premiums which is not the case for the patient-centered approach. Nekhlyudov et al. (2014).
Furthermore, loyalty and trust are essential between the patients and healthcare professionals, which are fostered by good communication platforms. When communication is poor between the two parties, patients may not feel free to express their problems fully. This forces the healthcare practitioners to make uninformed decisions leading to healthcare problems like misdiagnosis, wrong medication, etc. the core benefits of increasing the involvement of patients when making healthcare decisions and evidence-based practices include; improved healthcare results since the plan of care is tailored to meet patient's unique problems. Secondly is better decision-making when patients are involved in making decisions of their healthcare course, e.g., their allergies, etc. The patient's satisfaction and the overall quality of the healthcare system are improved through a patient-centered approach since hospital re-admission and admission period reduce gradually. Sidani et al. (2006).
However, several limitations exist when applying the patient-centered approach. Firstly is due to the patient's limited information, which may limit the communication process. Additionally, the caregivers and family members may have limited information regarding the patient. The second problem is the lack of time when healthcare professionals are often engaged in other activities due to low staffing. Therefore addressing the specific needs of each patient is often difficult.
Some remedies to these challenges include; firstly, the EHRs are a good alternative where patients' records are stored electronically for healthcare workers to retrieve and apply. It's a good option when patients do not adequately understand their healthcare issues for HealthCare workers to detect their allergies and treatment history. The second problem of poor nurse-to-patient ratio can be solved through healthcare facilities limiting the number of patients they admit based on their staffing levels. This will help to reduce nurses being overwhelmed with work. Secondly, healthcare facilities can employ new practitioners to deal with increasing numbers of patients. Kent et al. (2012).
