I have a care plan scenario im trying to find the best nursing diagnosis and submit a care plan for the scenario is.

Mrs. Wase is 85 years ol has a history of osteoporois, lives alone and self medicates. She takes BP medication due to her hypertension. She remains alert and oriented x 2 but she has dizzy spells at times she has a walker but "uses it on her bad days" as she says. some days she has weakness in both legs. Thats when she remembrs to take her calcium and vitamin D3. Her family checks on her everyday by a visit or phone call.

I have looked into impaired physical mobility and also risk for fall and noncompliance. can you help me form this care plan properly
