This is a continuation of the SSD proposal assignment that you completed earlier in the semester. To complete this assignment, first review your proposal, make needed edits based on feedback and then add the remaining content.

Sample: describe the sample (YOU are the sample and must minimally present info about your age, gender, race, and any other aspects that may be relevant to this study).

Problem: Describe the problem you addressed in detail including the consequences of the problem for you.

Intervention: What was the original intervention supposed to consist of? How did the actual vary intervention vary from the original plan? If it changed, when did it change and why did you change it? If there was even a small change in the intervention, you must discuss it here. If you conducted it as planned, what do you think contributed to your ability to carry out the intervention in the proposed manner?

Measurement: Instrument identification: What instrument did you use to measure the effectiveness of your intervention? In retrospect, do you believe this was the most effective way to assess the intervention? If not, what would have been a more effective way to assess the intervention?

Measurement points: You must provide both a table and a graph of all measurement points you collected. Both the table and graph must include the date and the score on your measurement instrument. The graph must be a line graph similar to that shown on p. 89 of your text.

Assessment of intervention: Based solely on the measure you used, was the intervention successful? What evidence is there to support your perception?

Secondary benefits: Did you receive any additional benefits from conducting the intervention, regardless of whether the intervention was effective based on the measure you used? If not, do you think you would receive any secondary benefits if you were to change the intervention and/or measures? If you did receive them, what were the secondary benefits and why do you think you received them?

What would you do differently or the same? Identify a total of three things you would do differently or the same with this SSD. Why would you do them differently or the same?

Lessons learned: Identify three things you have learned in conducting interventions and measuring them and how each will assist you in your practice with clients.

Self-care lessons learned: identify at least two things you have learned about self-care in the course of completing this assignment and how they are relevant to your future social work practice and/or your life in general.
