GETTING STARTED

• Open the file NP_AC19_EOM6-2_FirstLastName_1.accdb, available for download from the SAM website.
• Save the file as NP_AC19_EOM6-2_FirstLastName_2.accdb by changing the “1” to a “2”.
  • If you do not see the .accdb file extension in the Save As dialog box, do not type it. The program will add the file extension for you automatically.
• To complete this SAM Project, you will also need to download and save the following data files from the SAM website onto your computer:
  • Support_AC19_EOM6-2_HeartLogo.jpg
• Open the _GradingInfoTable table and ensure that your first and last name is displayed as the first record in the table. If the table does not contain your name, delete the file and download a new copy from the SAM website.

• PROJECT STEPS

1. Cardiac Rehab Associates is a company that helps people rehabilitate their health and strength after an illness or an accident. You work as a research analyst to maintain records and to analyze the success of various activities and programs. At Cardiac Rehab Associates, you are developing an Access database application to help manage this information. In this project, you will improve the existing database by formatting, modifying, and creating new forms. You will also modify several different types of form controls.
   
   Create a split form from the Clients table. Save the form with the name ClientSplit and close it.
2. Create a Multiple Items form from the InsuranceCompanies table. Save the form with the name InsuranceMultiple and close it.
3. Open the ClientEntry form in Design View and change the Record Source for the form to the ClientListing query. Change the tab order so that the StartDate field is in the last position in the Detail section. Save the ClientEntry form.
4. In Design View of the ClientEntry form, use the Combo Box Wizard to add a combo box to at about the 5" mark on the horizontal ruler in the Form Header section to find a record. Select the LastName and FirstName fields for the combo box in that order. Do not adjust column widths. Enter Find Client as the label for the combo box. Save the ClientEntry form. Switch to Form View and use the new combo box to find the Albin, Pedro record.
5. Open the HealthProgramEnrollment form in Design View, add a check box control below the Fee text box. Modify the Control Source property of the check box to the Over18 field and use Must be 18 or older? as text of the accompany label. (Hint: if necessary, change the width of the label so that the full text is visible). Save the form and switch to Form View as shown in Figure 1, then close the HealthProgramEnrollment form.

6. Open the ClientInfo form in Design View. Select the six controls (three labels and three text boxes) in the Detail section and apply a Stacked Layout to them. Expand the Detail section to be about 3" tall. Use the Subform Wizard to add the EnrollmentDate field from the Enrollments table and the ProgramName and Fee fields from the HealthPrograms table in the subform. Show Enrollments for each record in Clients using the ClientID field. Use the default name for the subform. Position the subform just below the LastName label. Save the form and switch to Form View as shown in Figure 2, then close the ClientInfo form.

7. Switch the ClientEntry form to Design view, and expand the Form Footer section to about 0.75" tall. Add a text box to the top right corner of the Footer section, and enter the formula =Count([ClientID]) as its control source. Enter Total Clients as the label name. (Hint: if necessary, change the width of the label so that the full text is visible). Add another text box directly below the one you just added, and enter the formula =SUM([MonthlyFee]) as the control source and Total Fees as its label name. Modify the Total Fees text box to use Currency format, then switch the ClientEntry form to Form view and confirm it matches Figure 3 below. Save the form.

8. Switch the ClientEntry form to Layout View. Add a new text box to the form and move it just below the StartDate text box. Use Fee as the Caption for the accompanying label and use MonthlyFee as the Control Source property for the text box. Save and close the ClientEntry form.
9. Open the InsuranceCompanyEntry form in Design View and right align the right edges of the Insurance Code and Company Name labels. Left align the InsuranceCode and CompanyName text boxes. Add a rectangle control around the four controls (two labels and two text boxes) in the Detail section. Save the InsuranceCompanyEntry form.
10. With the InsuranceCompanyEntry form still open in Design View, insert the Support_AC19_EOM6-2_HeartLogo.jpg logo image into the Form Header. Save the form and switch to Form View as shown in Figure 4 and then close the InsuranceCompanyEntry form.

Save and close any open objects in your database. Compact and repair your database, close it, and then exit Access. Follow the directions on the SAM website to submit your completed project.
