CREATING AN ONLINE FORM

  • *GETTING STARTED
  • Open the file SC_WD19_10b_FirstLastName_1.docx, available for download from the SAM website.
  • Save the file as SC_WD19_10b_FirstLastName_2.docx by changing the “1” to a “2”.
    • If you do not see the .docx file extension in the Save As dialog box, do not type it. The program will add the file extension for you automatically.
  • With the file SC_WD19_10b_FirstLastName_2.docx still open, ensure that your first and last name is displayed in the footer.
    • If the footer does not display your name, delete the file and download a new copy from the SAM website.
  • To complete this project, you will need to display the Developer tab.
    • If the Developer tab does not display, click the File tab to open Backstage View and then click the Options button. In the Word Options dialog box, click Customize Ribbon and then click the Developer check box. Click the OK button to close the Word Options dialog box and confirm the Developer tab appears in the Word ribbon.
  • PROJECT STEPS
  1. After working in the construction industry for a few years, you started Haven Home Inspection, a part-time business you run out of your home in Des Moines, Iowa. When potential customers inquire about your services, you want to send them a survey, which you are developing now. You plan to send the survey to customers via email, and then ask them to fill it out and return it to you the same way. You need to complete the survey as an online form in a Word document.
Change the paper size to a width of 8" and a height of 6" to reduce the amount of white space at the bottom of the page.
  2. Change the page color to Light Gray, Background 2, Lighter 80% to make the form more attractive.
  3. Add a content control for entering a last name as follows:
    1. In the second cell of row 1, insert a Plain Text content control after the "Last Name:" text.
    2. In Design mode, edit the placeholder text so that it appears as follows:
Click here to enter your last name.
    3. Format the new content control using the Strong style.
    4. Lock the content control so that it cannot be deleted.
  4. Add a content control for selecting how customers heard about Haven Home Inspection as follows:
    1. In the second cell of row 3, insert a Check Box content control to the left of the space.
    2. Change the properties of the check box to use Website as the tag.
    3. Choose a setting so that no border is displayed around the check box.
    4. After the space, enter Website as the label for the check box.
  5. Add a content control for specifying another way to hear about the company as follows:
    1. After the text "Other (please specify):" in row 4, insert a Rich Text content control.
    2. In Design mode, edit the placeholder text so that it appears as follows:
Click here to list other ways you heard about us.
  6. Add a content control for selecting a role in the home inspection as follows:
    1. After the text "What is your role in the home inspection?", insert a Drop-Down List content control.
    2. Do not allow the drop-down list content control to be edited.
    3. Remove the "Choose an item." entry.
    4. Add the following three choices for the drop-down list:
      Buyer
      Seller
      Realtor
  7. Add a content control for indicating the location of the home as follows:
    1. After the text "Where is the home located?", insert another Drop-Down List content control.
    2. Do not allow the drop-down list content control to be edited.
    3. Remove the "Choose an item." entry.
    4. Add the following four choices for the drop-down list:
      Downtown
      East
      West
      South
  8. Add a content control for selecting a date as follows:
    1. After the text "Today's date:", insert a Date Picker content control.
    2. Select the date format similar to November 20, 2021.
    3. In Design mode, edit the placeholder text so that it appears as follows: Click here to enter a date.
  9. Remove all the borders from the table so the top part of the form blends in with the bottom part of the form. (Hint: Make sure to select the entire table when removing borders.)
  10. Insert a rectangle to separate the form from the page background as follows:
    1. Draw a Rectangle: Single Corner Snipped around the form text, from the "First Name:" line to the "Today's date:" line.
    2. Change the height of the rectangle to 2.4" and change the width to 7.5".
    3. Move the rectangle so that its top and bottom edges encompass the form text, and then align the rectangle to the center of the page.
    4. Apply the Subtle Effect – Teal, Accent 5 shape style to the rectangle.
    5. Apply an Offset: Bottom shadow to the rectangle.
    6. Send the rectangle to behind the text.
  11. Restrict editing in the document to allow users only to fill in forms, and start enforcing protection without a password.

Your document should look like the Final Figure on the following pages. Save your changes, close the document, and then exit Word. Follow the directions on the SAM website to submit your completed project.
