  • Open the file SC_WD19_EOM10-1_FirstLastName_1.docx, available for download from the SAM website.
  • Save the file as SC_WD19_EOM10-1_FirstLastName_2.docx by changing the “1” to a “2”.
    • If you do not see the .docx file extension in the Save As dialog box, do not type it. The program will add the file extension for you automatically.
  • With the file SC_WD19_EOM10-1_FirstLastName_2.docx still open, ensure that your first and last name is displayed in the footer.
    • If the footer does not display your name, delete the file and download a new copy from the SAM website.
  • PROJECT STEPS
  1. You are an administrative assistant at the Santa Fe Film Office, an agency for the City of Santa Fe that grants permits for filming within the city. You are developing a form to send to film producers via email, and plan to ask them to fill in the form and then return it to you the same way.
    Change the paper size to a custom size using a width of 8.2" and a height of 5.8" so that users do not need to scroll when completing the form online.
  2. Change the page color to Gold, Accent 6, Lighter 40%, and then apply the Divot pattern (7th column, 1st row in the Pattern palette) to make the form more attractive.
  3. Insert the clapperboard icon (6th column, 1st row in the Arts category) shown in Figure 1 to add visual interest to the form. Position the icon in the top right with square text wrapping. Apply the Colored Fill—Accent 4, Dark 1 Outline graphics style to the icon.

  5. In the first cell of row 1, insert a Plain Text content control after the "Project Title:" text. Format the new content control using the Subtle Emphasis style, and then lock the content control so that it cannot be deleted. In Design mode, edit the placeholder text so that it appears as follows:
    Click to type the project title.
  6. In the first cell of row 3, insert a Check Box content control to the left of the space. Change the properties of the check box to use TV as the tag and to display no border around the check box. After the space, enter TV as the label for the check box.
  7. After the text "Other (please describe):" in row 4, insert a Rich Text content control. In Design mode, edit the placeholder text so that it appears as follows:
    Click to describe your project type.
  8. After the text "City services requested:", insert a Drop-Down List content control. In Design mode, use Click to select a service. as the placeholder text. Do not allow the drop-down list content to be edited. Remove the "Choose an item." entry, and then add the following three choices for the drop-down list:
    Close street
    Restrict parking
    None
  9. After the text "Special effects planned:", insert another Drop-Down List content control. In Design mode, use Click to select a special effect. as the placeholder text. Do not allow the drop-down list content to be edited. Remove the "Choose an item." entry, and then add the following four choices for the drop-down list:
    Fire
    Rain
    Snow
    Wind
  10. After the text "Project start date:", insert a Date Picker content control. Select the date format similar to 16-Jan-21. In Design mode, edit the placeholder text so that it appears as follows:
    Click to enter the start date.
  11. Remove all the borders from the table so that the top part of the form blends in with the bottom part of the form. (Hint: Make sure to select the entire table when making changes to the table border.)
  12. Draw a Rectangle: Single Corner Snipped around the content in the document. Change the height of the rectangle to 2.7" and change the width to 7.8". Move the rectangle so that its top and bottom edges encompass the table, content controls, and last paragraph, and then align the rectangle to the center of the page. Apply the Colored Outline—Gold, Accent 6 shape style to the rectangle. Apply an Offset: Left outer shadow to the rectangle. Send the rectangle behind the text.
  13. Restrict editing in the document to allow users only to fill in forms and start enforcing protection without a password.
  14. Your document should look like the Final Figure on the following pages. Save your changes, close the document, and then exit Word. Follow the directions on the SAM website to submit your completed project.
