Scott is a very active 12 year old boy. He decides he is going to climb higher than
anyone else on the tree. Scott is a good 30 feet up in the air. His friends are begging him
to come down. Scott is almost to the very top of the tree when the branch breaks. He his
several branches on the way down and lands on the sidewalk. His friends rush to him.
He is unconscious. One of his friends calls 911. When the EMTs arrive, they examine
Scott and though Scott is unconscious, his vital signs are stable. One of Scott's friends
asks the EMT what is the pink fluid coming out of Scott's ear? The paramedics are
working fast. They arrive at the hospital in record time. Miraculously, Scott wakes up
on the way to the hospital. He is a very good patient and holds still for a head CT.
Scott has fractures on both his right temporal and sphenoid bones. You exam Scott's
chart and notice the following observations:
? Observation 1: Scott has double vision (diplopia) and his right eye is turned
slightly inward when looking straight ahead.
? Observation 2: Numbness of the right upper lip, lower eyelid, and cheek
Observation 3: Paresthesia (sensation of pins and needles) at the right corner of
the mouth, extending to the lower lip and chin
? Observation 4: Complete loss of hearing in the right ear
Each of the four observations on Scott's chart indicate damage to a cranial nerve.
