Describe your workplace procedures for documenting the outcome of the assessment process. Attach evidence that you have documented the outcomes of the assessment process for this person.
Describe your workplace procedures for maintaining and storing client information, as well as in regards to confidentiality. Attach evidence that you have maintained and stored client information regarding confidentiality requirements.
Describe how you provided each person's information to other services according to consent and confidentiality requirements.
Describe feedback you received about the assessment processes use both from the person and others involved in the assessment process. You could also attach any specific evidence related to this such as completed surveys.
Describe how you monitored the assessment processes and outcomes to assess their success in terms of meeting the person's needs.
Explain the processes you used to seek feedback on your work. What feedback did you get? What does the feedback tell you about your performance in assessing the co-existing needs of each person?
Based on feedback and your own evaluation, describe at least one opportunity for improving processes.
