Social Ecological Model FINAL Assignment
Purpose: An ecological model is way to show how an individual is inter-connected with various aspects of their environment, from interpersonal to organizational, to community, to public policy. The purpose of this final assignment is for you to demonstrate your understanding of the interplay between the levels of the SEM. This is also integrating some of the knowledge from the lesson on social determinants of health. While social determinants of health are the systems in which we grow, live, work, and play, it has an impact on the different levels of the social ecological model that ultimately will impact our health.
Directions: Before completing this assignment, make sure to view the SEM lecture and read the Baral et al. (2013) article on the “Modified social ecological model” and the Kalinowski et al. (2019) article on “Why are young Black women at high risk for cardiovascular disease.” Review your SEM assignment I, II, and III feedback. Use that feedback to improve upon your model for this assignment.
Cardiovascular disease is one of the top ten leading causes of death in the United States and Black women are disproportionately affected. Some modifiable individual risk factors for cardiovascular disease include diet (high salt intake or high red meat intake), physical activity (sedentary lifestyle), exposure to smoke (smoking status or secondhand smoke), stress and alcohol use. For more information and ideas about heart disease and prevention please visit the CDC prevent heart disease page.
1. Select ONE modifiable behavioral factor you would like to change that could impact Black women’s risk for cardiovascular disease. Write it in the box under “Modifiable Risk Factor.”

3. Please list 2 factors for EACH level that could improve the MODIFIABLE BEHAVIORAL FACTOR you selected in #1. These are potential intervention points. Describe under the Intervention column how you would intervene or change the factor you identified. These can be bullet points but provide enough information such that we can see how the intervention or activity will change the related factor that would impact the modifiable risk factor. The first row is an example.
