Patient T seemed alert and awake throughout the therapy session. He transitioned into the session without difficulty. He was highly participative and engaged in every present speech activity.T's teacher commented that he had understood T better in the previous week.
Objective (O)
T produced /r/ at the start of words with an accurate level of 70% using a worksheet with 10 /r/ vs./l/ minimal pairs, provided two verbal reminders; for instance, "Is it rate or late?") and a self-correction mirror.
Assessment (O)
T's correct production of /r/ in the first place improved from 60% in imitation and visual aid during the final session to 70% in the current session. Omission for visual assistance that involves a picture of mouth producing /l/ failed to lower performance.
Plan (P)
T continues to enhance her production of initial /r/. Similarly, a patient continues to fade cues and initial sound model for initial /r/ at the subsequent therapy. She was sent home with four minimal pair worksheets from the JKL reading material and directed T's caregiver on the best ways for completing.
