SUBJECT: Request for Religious Accommodation Exemption from COVID-19 Vaccination 1. I respectfully request a religious exemption from the COVID-19 vaccination mandated by the Secretary of Defense on 24 AUGUST 2021 on the grounds of religious accommodation. I make this request for the following reasons. a. This request is based on the premise and principle that I firmly and undoubtedly believe in my Non-denominational Christian guiding morals to respectfully object this executive order rendered by the President of the United States. I understand that by taking the oath of enlistment, I am obligated and required to comply with and obey lawful and direct orders per the enlisted force structure; however, according to my faith, the matter of the COVID-19 vaccination mandate is where I have to raise just cause for my spiritual concern. 2. I have read and understand Department of the Air Force Instruction (DAFI) 52-201, Religious Freedom in the Department of the Air Force, and I have been counseled by all necessary individuals. After fully considering the matter, I respectfully make this request.
