Describe an intervention to solve the problem. for example Patient education, adding clinical resources, innovative care management scheme. for the intervention, explain the following:

a) Who is the target of the program or intervention

b) What is the program (list all components...who implements it? what steps are involved?)

c) Explain the outcome/goal that will be reached by the program/intervention

4) Health Policy

Propose a pathway for changing health through addressing a health policy or bill related to the problem. Identify a related health policy or bill. This should be a real bill or policy within the last 2 years and should have some influence on the problem. It can be very broad like Medicaid expansion, or improved housing and also Include the following:

1. Describe if the policy is at the national or state or community level.
2. Describe what you see is needed to make this policy or bill work toward improving health. Are you suggesting that a bill be amended? Are you wanting to have a representative sponsor a new bill? Does it exist somewhere in the legislative process but needs more support? What is your plan? List the steps you will take to move this policy or bill forward.
